Description:
In
this course, you will review the key components to a Patient-Centered
HealthCare Home (PCMH) and the essential principles which create a patient and
family-centered experience. Practices and strategies which emphasize integrated
and coordinated care will be discussed, such as a team-based care approach, use
of health information technology and data, using Quality Improvement (QI)
performance measures, and the rationale for primary care payment reform that
rewards for quality rather than quantity.

- Discuss
a real-world case study of a healthcare home focused on transforming their patient
care, and the strategies and practices utilized at the home to attain more
integrated and coordinated care.
